I don't see the Americans doing it - at the time of the Revolution, were the British using squares? Formations, maybe, but not squares.

Well, what do you define as mob? During the Sudan campaign 1881-99 it happened more than once, e.g. Tamai 1884 and Abu Klea 1885 by the Mahdist Army, which was at least a bit of a mob.
But we also saw breaks during the Napoleonix wars, like Rio Seco 1808.

The name of the award is of little consequence. The VC and the MOH are the top, and nothing can change their significance.
The film 'ZULU' is a good one that depicts the events in a believable manner.
